| TOWNSHIP SERVICES:
General Assistance – General Assistance
is a monthly assistance program that assists with living expenses
and may include medical assistance. Applicant must be a township
resident and meet the guidelines set forth by The Township Officials
of Illinois.
Emergency Assistance – Emergency Assistance
is an annual assistance program that helps residents alleviate a
life-threatening circumstance or to assist in self-sufficiency.
Some circumstances may include eviction or past due rent or mortgage,
utility disconnection, or prescription assistance. Applicant must
be a township resident and meet the guidelines set forth by The
Township Supervisor.
Food Pantry – We operate a food pantry which
assists township residents who are experiencing financial difficulty
or on a fixed income with non-perishable food products. You must
be a township resident to receive items. The food pantry is open
during normal business hours but recipient should call to schedule
a pickup time.
We accept donations of non-perishable food items, paper products,
cleaning supplies, toiletries, grocery store gift cards, and cash.
Giving Garden – We accept donations of fresh
produce to distribute to our less fortunate residents. Donations
can be dropped off at the township office during normal business
hours.
LIHEAP – LIHEAP is a low-income heating
assistance program funded through the State of Illinois. It is yearly
program that assists residents with funds to pay for their heating
bills. The income guidelines are as follows:

You will need social security cards for all members of the household,
proof of income for all eligible members of the household, current
gas and electric bills, lease (if applicable), and current Medicaid,

ALLKIDS / KidCare / FamilyCare – Kidcare
/ FamilyCare is a health insurance program for Illinois families.
If you are pregnant and a resident of Illinois, or your children
are 18 years old or younger and are Illinois residents and U.S.
citizens and you meet the income guidelines, then you might qualify
for this program. Applications are available at the township office
and our staff can assist in filling out the application. www.kidcareillinois.com
Public Aid / LINK Applications – We have
Illinois Department of Human Services applications available in
English and Spanish. Programs available are food stamps, medical
cards, and Cash Assistance. We will assist you with your application
and will fax in your completed application. www.dhs.state.il.us
Share-the-Harvest Program – Donators provide
groceries or grocery store gift cards to make a Thanksgiving meal
for less fortunate township residents. We have seniors and families
to provide for. Please call if you would like to donate or if you
would like to be a recipient.
Adopt-A-Family Holiday Program – Donators
provide groceries or grocery store gift cards to make a Holiday
meal, or purchase gifts for less fortunate township residents. The
township will give the donator gift ideas and sizes to make shopping
easier. Gift cards are always welcome. Please call if you would
like to donate or if you would like to be a recipient.
Social Service Agency Referrals – If you
have a problem that we cannot assist with, we can refer you to the
appropriate agency.
AARP Driver Safety Class - This class is designed
for drivers age 55 and over. Topics include: How to drive defensively,
deal with agressive drivers, and how to adjust to vision and hearing
loss.
Benefit Access (formerly Circuit Breaker) - Benefit
Access is a program available through the Illinois Department of
Aging for seniors and disabled. This program may help with a license
plate discount. If you qualify for this program, you are eligible
for a RTA / METRA Free Ride Pass. You have to apply online for this
program. Our staff will assist with filling out the application

Pace - There is a PACE Bus available for senior
and disabled residents. The bus runs on Tuesdays and Thursdays to
select shopping areas in Lake County. For a ride on Tuesday, call
on Monday. For a ride on Thursday, call on Wednesday. Call 847.223.6300
between 10:00 a.m. and 3:00 p.m. to schedule a ride. The fare is
$1.00 for a one-way trip. www.pacebus.com
Pace Northwest Demo Project - This is a Dial-a-Ride
service that operates on Monday, Wednesday, and Friday from 9:30
until 2:30. Residents may go to any destination within the Fremont
and Wauconda township boundaries. Other destinations include:

Mid-Lakes Clinic (Lake County Health Dept.) - Round Lake Beach
Seniors and Disabled are given first priority but this service
is open to the General Public. One-way fare is $2.00 for Seniors
and Disabled and $4.00 for the General Public. An application must
be filled out and be on file with the township.

RTA/METRA Reduced Fare Pass – Applications
are available to receive a Senior / Disabled Reduced Fare RTA/METRA
Pass. You must be 65 years of age or disabled and bring in an Illinois
State I.D. or Drivers License. If disabled, there is a section on
the application for the physician to fill out or if you are receiving
Social Security Disability, you need to bring in your Notice of
Award letter. We will take your picture free of charge. It normally
takes 3 weeks to receive your pass. The Free Ride Pass is
for Benefit Access recipients only. Schedules are available

Handicapped Parking Placard Applications –
Applications to receive a permanent or temporary handicapped parking
placard are available at the township office. You can bring your
completed application to our office and we will fax it to the Secretary
of State’s office.CLICK

Notary Public – We will notarize your documents
free of charge.
Township Maps – Township maps are available
free of charge.
Lake County Prescription Discount Card –
This prescription card is for individuals without any prescription
coverage or for medicine that your current plan will not cover.
It is available to any Lake County resident and free of charge.
Mosquito Control & Abatement – The township
has contracted with Clarke Mosquito Control to provide mosquito
control and abatement.
Healthreach Clinic – Healthreach is a medical
clinic for the uninsured. It is located at 111 N. Seymour Avenue,
Mundelein, IL. 847.360.8800. Fremont Township subsidizes this clinic.
Parks & Recreations Mundelein Baseball & Softball
Association – All township residents may register
for the Mundelein Baseball & Softball Association program without
paying out-of-district fees. This is due to a Community Partnership
between the Little League and the township. For more information,
call the Little League at 847.949.0510.
Ivanhoe Community Club Park – This park
is located on Route 83 ½ mile past the intersection where
Routes 83 & 60 split in Mundelein. This park has a ballfield
with lights for night games. Currently, it is being used by the
Mundelein Baseball & Softball Association. The Ivanhoe Community
Club donated this park.
Behm Homestead Park – This park is located
on Peterson Road ½ mile east of Route 60. Ammennities include:
baseball, softball, football, soccer, and lacross fields. Walking
trail with fitness stations and a playground.

NICOR Sharing Program - This program provides
energy assistance for NICOR customers only. You must meet the income
guidelines and did not receive a LIHEAP grant in the same fiscal
